This printable questionnaire is designed for first-day choir class. Students answer nine open-response questions about their names and pronouns, birthdays, favorite colors, preferred school-appropriate songs, ensemble participation, and what they're excited about for the year. Useful as an icebreaker activity, to build community in a high school music ensemble, or to gather background information on incoming students.
